อ่านต่อUveal melanoma is the most common primary intraocular malignancy in adult. The incidence of uveal melanoma is more common in Caucasian than in Asian population. Uveal melanoma is categorized into 3 types with regard to the parts of the eye; the choroid, which is the most common one, ciliary body, and iris. One of the symptoms of choroidal melanoma invariably includes blurred vision. Risk factors for development of choroidal melanoma include Caucasian race, sun exposure, oculodermal melanosis, and preexisting choroidal nevus.

อ่านต่อProliferative vitreoretinopathy (PVR) เป็นสาเหตุสำคัญของการล้มเหลวของการผ่าตัดรักษาโรค retinal detachment ซึ่งเกิดจากการที่มี cell migration และเกิดการรวมตัวของ collagen สร้างขึ้นมาเป็น membrane ทั้ง inner และ outer surface of retina โดยเมื่อ membrane เกิดการหดตัวขึ้นทำให้เกิดเป็น retinal fold
อ่านต่อโรคบางโรคที่เป็นโรคง่ายๆ ทำให้เรามองข้ามโรคร้ายแรงที่ซ่อนอยู่ก็เป็นได้ ตัวอย่างเช่น ภาวะ astigmatism ผู้ป่วยอาจจะมี ciliary body melanoma ซ่อนอยู่ ภาวะ chalazion อาจจะเป็นได้ทั้ง sebaceous gland carcinoma, basal cell carcinoma, Merkel cell carcinoma, metastasis, sweat gland neoplasms ภาวะ pterygium อาจจะเป็น CIN หรือ conjunctival malignant melanoma ภาวะ strabismus, leukocoria อาจจะเป็น retinoblastoma เป็นต้น
อ่านต่อการรักษา myopic CNV ประกอบด้วย 1. Laser photocoagulation: ข้อเสียคือ มีโอกาสกลับมาเป็นซ้ำได้สูง ไม่ช่วยให้ระดับการมองเห็นดีขึ้นได้ในระยะยาว และเกิด laser scar expansion ในภายหลัง 2. Photodynamic therapy: ข้อเสียคือ ไม่ทำให้การมองเห็นดีขึ้น และเกิด chorioretinal atrophy ได้ 3. Anti-VEGF: มีการศึกษาพบว่าการฉีด intravitreal anti VEGF เพื่อรักษา myopic CNV สามารถช่วยให้การมองเห็นดีขึ้นมากกว่าผู้ป่วยกลุ่มที่ได้รับเฉพาะ PDT โดยที่ intravitreal anti-VEGF ช่วยลด central foveal thickness และป้องกัน irreversible retinal damage
อ่านต่อPCV พบได้บ่อยกว่าในคนเอเชีย เป็น variant ของ type 1 CNV และเป็น subtype ของ nAMD มีลักษณะ คือ abnormal BVN และ polyps ปัจจุบันการรักษาที่เป็นมาตรฐาน และให้ visual outcome ที่ดี คือ การใช้ Anti-VEGF จึงมีการค้นคว้าวิจัยมากมายเกี่ยวกับ Anti-VEGF โดยเปรียบเทียบกับการรักษาด้วยวิธีอื่น และในกลุ่ม Anti-VEGF ด้วยกันเอง
อ่านต่อเครื่อง iOCT สามารถช่วยแสดงผลของการผ่าตัด membrane peeling ได้ โดยแสดงให้เห็นว่าการลอกยังทำไม่ครบถ้วนได้ถึงร้อยละ 22 และในทางตรงข้ามก็ได้แสดงให้เห็นว่าร้อยละ 15 ของเคสที่จำเป็นที่จะต้องลอกนั้น ได้มีการลอกที่ครบถ้วนแล้วไม่จำเป็นที่จะลอกต่อ นอกจากนี้ในแง่ของการซ่อมแซมการหลุดลอกของชั้น retina เครื่อง iOCT สามารถช่วยหาจุดที่จะทำการระบายได้อย่างเหมาะสม ช่วยหา dissection plane หรือ retinal break และยังช่วยตรวจหา full-thickness macular hole ได้

อ่านต่อNeuroimaging ในผู้ป่วย NTG เพื่อค้นหา space occupying lesion (SOL) ควรทำในกรณีต่อไปนี้ คือ disc pallor out of proportion to VCDR, field changes out of proportion to disc changes, grossly asymmetrical glaucoma, too fast progression rate, very young patient, decreased color vision/RAPD/neurological symptoms like headache
